2024 Program Forms

Please complete and return the following forms to sign up for Achieve Tahoe’s Programs.

2. Liability Releases 1 – 7*

*If you have already filled out forms for our winter programs this 2024 season, you only need to fill out Liability Releases 5 – 7

Once you have completed all your forms, you will be sent a link to sign up. If you are signing up for 2024 summer sports, you will be sent a link to sign-up starting April 24th, 2024.
